COMMUNITY HEALTHCARE WORKER

We are seeking a highly organized, proactive, and compassionate Community Healthcare Worker (CHW) to join our pediatric practice. This role is IN-PERSON and supports children, adolescents, and families by identifying and addressing social determinants of health (SDOH) that impact wellbeing, treatment adherence, development, and access to care. The CHW will work closely with providers, behavioral health staff, and families to connect patients with community resources, coordinate care across systems, and remove barriers related to housing, food insecurity, transportation, insurance, behavioral health access, and other family stressors. This position is ideal for someone who thrives working independently, takes initiative without needing close supervision, and can manage a high volume of moving parts with exceptional attention to detail. 

 Key Responsibilities: 

  • Conduct SDOH screenings and assess family needs through in-person, phone, and virtual outreach 
  • Provide resource navigation for housing, food, transportation, utilities, childcare, insurance, and financial assistance
  • Coordinate referrals to community agencies and specialty services
  • Track referrals, outcomes, follow-up tasks, deadlines, and family engagement with meticulous documentation in EHR (Epic) 
  • Collaborate with pediatricians, nurses, therapists, and on-site Educational Consultant to ensure coordinated care 
  • Maintain updated knowledge of local community resources and eligibility requirements
  • Assist families in completing forms, applications, and connecting to services
  • Escalate urgent safety or psychosocial concerns to appropriate clinical staff promptly
  • Identify gaps in systems/processes and independently develop solutions to improve workflow and patient support 
  • Participate in team meetings where appropriate and quality improvement initiatives
  • Communicate with English and Spanish-speaking clients regarding services, scheduling, and support.

 Requirements:

  • Bachelor’s degree in Social Work, Human Services, Public Health, Psychology, Sociology, or related field
  • Experience in care coordination, case management, healthcare navigation, social services, or pediatric/family support
  • Strong knowledge of SDOH and community resource systems
  • Excellent organizational skills with ability to manage multiple families and priorities simultaneously
  • Demonstrated ability to work independently, problem-solve, and take initiative 
  • Ability to travel between three pediatric locations, all within the North Shore
  • Strong written and verbal communication skills 
  • Comfortable working with diverse families in a fast-paced healthcare setting
  • Proficiency with electronic medical records (or a willingness to learn) and standard office technology
  • Experience in pediatrics, family medicine, behavioral health, or community health centers
  • Familiarity with Medicaid/MassHealth and insurance navigation 
  • Bilingual English/Spanish required due to client communication needs.

PATIENT CARE REPRESENTATIVE

(Full-Time Monday - Friday, 7:00 am - 3:30 pm OR Monday - Friday, 7:30 am - 4:00 pm)

Working under the supervision of the Operations Manager, the Patient Care Representative is responsible for managing high volumes of inbound and outbound calls, assisting patients with scheduling, registration, and general inquiries, and supporting front desk operations. This role ensures smooth communication between patients and clinical staff while maintaining a professional and welcoming environment.

Key Responsibilities: 

  • Greet patients and families at the front desk upon entering the office
  • Answer and route incoming calls promptly and professionally
  • Schedule, confirm, and reschedule patient appointments using electronic health records
  • Receive and respond to messages via voicemail and Patient Portal for urgent and non-urgent appointments
  • Verify patient demographic and insurance information
  • Follow communication scripts and protocols for various patient interactions
  • Administrative duties and projects assigned as needed
  • Maintain confidentiality and comply with HIPAA regulations

Requirements:

  • Must be 18+ and have either a high school diploma or graduation equivalency degree (GED)
  • Experience with Epic EMR helpful
  • Visual/hearing ability sufficient to comprehend written/verbal communication.
  • Ability to perform tasks involving physical activity, which may include lifting supplies, equipment, or babies and small children; extensive bending, standing, walking or sitting may be required.

REGISTERED NURSE II

Under the Supervision of the Pediatrician, Nurse Practitioner and Nurse Manager, the Registered Nurse maintains smooth flow of operation, manages high risk and chronically ill patients, provides education to patients and families, provides direct care and laboratory services and is responsible for telephone and direct triage.

 Job Responsibilities:

  • Demonstrates basics of infection control including standard precautions
  • Demonstrate skills and knowledge directly related to age-appropriate care of pediatric patients as well as proficient knowledge of equipment and instrumentation
  • Assess the physical, emotional, psychosocial and socio-economical needs of the client and family, and refer to appropriate organizations for further evaluation when appropriate
  • Provide urgent triage care in the office as needed as well as urgent telephone triage and/or crisis management
  • Use EMR to completely and accurately documents nursing care including triage, education, medication, reconciliation, and care planning when appropriate
  • Act as facilitator of communication between family, physician, front desk and other health professionals
  • Administer vaccines and medications, and perform in-office procedures such as blood work and catheterization in an efficient and professional manner, delegate appropriate tasks to Medical Assistant and other staff when appropriate
  • Contact patients and families after Emergency Room ad Hospital discharge to prevent readmission and post-hospitalization complications. Coordinate with home care services if necessary
  • Maintain pediatric-centered continuing education and training
  • Adhere to the general policies and procedures as outlined by the practice
  • Ability to meet the necessary physical demands of the position

Educational Requirements:

Graduation from an accredited nursing program. The nurse requires a current license to practice nursing in the Commonwealth of Massachusetts by the Department of Public Health Nursing Certification Board as either a Registered Nurse (RN) or Licensed Practical Nurse (LPN). The nurse must also maintain Basic Life Support (BLS) certification and Continuing Educational Units (CEU’s) as required for licensure. S/He must also practice within the guidelines of the American Nursing Association Standards or Practice.


Medical Assistant

(Full-Time)

The Medical Assistant (MA) provides direct patient care to pediatric clients under the direction of the Registered Nurse, Licensed Practical Nurse, Nurse Manager, or Health Care Provider. This individual must be able to provide hands on care and perform all outlined duties appropriately and within the scope of practice. The MA is the flow manager of the team. The Medical Assistant is also responsible for monitoring referrals and providing telephone follow up after patients have seen an outside provider. S/he must have knowledge of office procedures and maintain communication with the other members of the care team.

Job Responsibilities:

  • Greet patients in a timely manner after check in and obtain necessary background information such as: reason for visit, vital signs, measurements, and prepare patient for physical examination by the doctor or nurse practitioner
  • Perform procedures such as: capillary blood testing, vision and hearing screenings, specimen collection, fluoride varnish application
  • Utilize pre-visit tools to collect patient data including current allergies and medications
  • Maintain sufficient stock of supplies in exam rooms and in the office; order medical supplies and equipment when necessary
  • Demonstrate basics of infection control including standard precautions
  • Accurately document vital signs, screening outcomes and other patient information
  • Maintain cleanliness of exam rooms and laboratory areas and prepare these areas for the next patient after they have been used
  • Monitor and track referrals to outside providers and services, follow up by phone with family necessary to ensure that the family has complied with MD/nursing recommendations
  • Provide age-appropriate and culturally sensitive care to clients and families
  • Adhere to the general policies and procedures as outlined by the practice
  • Ability to meet the necessary physical demands of the position
  • Maintain standards as outlined by the American Association of Medical Assistants (AAMA) or the American Registry of Medical Assistants (ARMA)
